Increasing affinity includes various actions such as gifting gift, assisting with quests and utilizing their services like hum.. puchasing goods or staying at their Inn etc..
If an NPC dislikes your gift, you're going to lose a bit of affinity altough it can be regaines quite easily with a bit of effort.
Certain NPC are easier because they have sign quest lines that can max out affinity with them.
Also, there was a special item called the Arisen's bond, it's a ring that you obtained in a quest and it played a key role in romancing your desired NPC specifically, it instantly maximized their affinity with you and i believe this item will also be in Dragon's Dogma 2.
If you had multiple NPC's with max affinity, the game at the end will automatically choose the last person with max affinity that you spoke to as you love interest.
This person then becomes tied to your arisen and becomes your partner in life.
I think, Dragon's Dogma 2 follows the same idea.

Romances with npcs are also confirmed.
In one of the reviews, someone mentioned something about choosing a npc (like the archer lady you meet when you first become arisen) to become the "heroine" of your story

Yes, according to Capcom some characters will have "episodes" if they have that, you will be able to romance them. Most likely those will be main characters.
If you'll be playing a female arisen your choices will be: two black males, one old white man, and an elf. If you'll play a male arisen your choices will be : two white women, one elf, and two furry females, perfectly balanced.
For other events, genders will play a role though. Just as being a beastren, people will react differently. But for romance I seriously doubt it. They showed early footage from IGN with Ulrika interacting with a female Arisen and she shows same interest.

We call that player sexual, where NPCS will just go with you no matter what. regardless if you're the opposite or same gender or the NPC's canon story. This has been the case in every game in the last decade with romance options, even skyrim didn't care.

Notice how there's no mention of gay relationships in my comment, did I say it will be restricted? You will not be able to romance anyone in the world, only specific characters will have romance quests. Most NPC's will most likely not.
Yeah that was funny funny stuff, "surprise your character is now gay!", wish they kept that for the sequel.
