I guess it IS a non issue. There's no real way to balance a NG+ given what the player is capable of and my conclusion is the game simply isn't made for it. It's fan service and/or sandbox mode where you customize your experience however you want.
I don't even need the carrots on the sticks, I just find the game fun to replay. Try going for the S+ ranks and see if you still find it easy.
That's kind of the point of NG+, and why they made it so you can never get S+ ranking when doing a NG+ playthrough, it's basically meant to be a victory lap for fun and for some other things like challenges/achievements that don't involve rankings/needing to be on a NG file like the Handcannon
I would agree adding in new bosses would have been an awesome thing to do sure, but that's a lot more work and I don't know what they could even add in given the game's pacing is already really good.
I do miss the U3 boss though. The design alone was terrifying even if it didn't really match the game very well.

I don't find the game easy, I think it's very well-balanced and challenging. Just a bit of buyer's remorse after getting the infinite rocket launcher since I feel that save is now more or less finished.
Realistically, if you want to play and keep the challenge just start a new save. Lets you work towards new rewards too that will, ultimately, also break the game and turn it into a power fantasy and victory lap ultimately, but you don't HAVE to use those things. If you want to try something truly challenging, try No Merchant Professional difficulty on a new save, its pretty wild. Also doing minimalist and frugalist at the same time is pretty challenging, though really, frugalist is a bit challenging regardless.
Edit: Also worth noting, while a knife run is probably possible WITHOUT infinite durability, it becomes a lot more feasible once you acquire one, and is something you could shoot for setting up and doing on a NG+ save.
the unlocks for the majority of items is from doing new game runs.
come back once you s+ everything (has to be done from new game) and have unlocked everything.
