Get the key to the church issue...SPOILER ALERT
I got both the Blasphemer's Head and the Apostate's head. I am at the alter where you place them on the two stone hands, but the prompts to do so are not showing up. Anyone have a similar issue?

Try changing your FOV settings and see if it helps.

Ah. Thank you. I am using the ultrawide mod and I had to turn it off to do that. That's so lame. Who knows what I've been missing. So frustrating.

To be fair developers don't bug test for mods, not really Capcom's fault.

Yea I get that but there shouldn't be the need for a mod like that in 2023. Every other AAA game manages to support UW, they can.
