Does the game only drop green herbs if you have low stocks of healing items?
I got like 6 green herbs all game long... maybe less. I had vials in my inventory with a rred and a yellow combined just waiting for a green, but I also had a stock of like 6 first aid spray. So are green herbs really rare, or is their drop rate dependent on what you have in your inventory?

Same question with handgun ammo... that would be good to know before starting hardcore. Does the game just give you ammo if you're out, so there's no reason to stock up?

more or less, boxes are the most likely to drop what you need while enemy drops are so-so. Dont know the math but the original did it too. Enemy's pretty much will only drop handgun or green herb if your very low, boxes seem to be a bit more generous and drop other ammo types and pretty much without fail a green herb if your completely out. You shouldn't rely on it however as it alone doesn't keep you stocked up enough for hardcore. As for guesstimate math it seems that 1 full restore is enough to drop the chance significantly and any more than that it drops the green herbs chance to very rare. handgun ammo is pretty much less than half-ish a mag for the chance to max. Other ammo types seem only slightly effected if you have low to no ammo (rifle, shotgun, etc) and only from boxes (in my experience) only have had enemy's drop other ammo types like twice could be I don't notice but its rare to non-exsistant.

TL;DR: Yes healing item drops are tied to the number of healing items you have.

I dunno if I'm a huge fan of that design decision but I guess it makes it feel like you're always low on ammo/healing/etc without ever letting you run out. So maybe it's an accessibility thing.

Could be wrong but think it was due to the change of pace, you have to kill enemy's in re4 and a lot of them, so it's a middle ground between survival horror and action. Dead space 1-2 and remake do it as well. so it's less DO I kill this enemy and more WHAT/HOW do I kill this enemy. While as you said retaining that resource hunger. Also original re4 had infinite knife so being empty completely was technically a non-issue.

For hardcore using your non pistol weapons (except bolt thrower) sparingly is very helpful and should be treated as a standard survival horror resource, while your pistol is the workhorse for everything else cause you get the most ammo for it and will never run out for long. The knife has been repurposed for the exact same thing but for health, negating damage, canceling grabs etc.

This game's design isn't made for stocking up, they want you low for that feeling of desperation, I imagine playing on normal though screws this up and you'll have healing items too long.

Store your first aid sprays away. Maybe carry one for emergencies.

You might be surprised how many survival horror games have had either that exact system or a variation of it baked into them over the many years of the genre existing.
Have ALL of them had it? Probably not, but I'd say more than you might think have had something like it at the very least.

I've been looting stuff for 30min and have only been getting coins and other trash, eventhough my healthbar is completely empty.

Replace the gold case with a default one.
