All S+ requires NG and can not be obtained with NG+
Saves don't affect ranking on any other difficulty than professional. I've gotten an S rank on a file with 100 + saves. 5:56 is over ranking requirements.
You are playing NG+ which drops the + from S rank and the requirements for S rank is 5:30 on Hardcore and Professional. A rank sounds about right.
thank you for this response, what about if I play Hardcore S+ does that cover Assisted and Standard as S+ too? marking it off?
Depends what you mean by marking it off? the game won't show you on the in-game ranking screen that you have earned S+ for Assisted and Standard, but getting S+ on hardcore will earn you all the achievements and challenges for lower difficulty S+ ranks too.
I'm pretty sure that's incorrect. I didn't unlock any other lower difficulties' achievements or unlocks when I did S+ hardcore. But I did unlock everything when getting S+ Pro.

The hell? Are you sure that's not just bugged out? Why on earth would they design it so that Hardcore S+ doesn't unlock lower difficulties but Pro S+ does? That's very unintuitive. I admit I skipped Hardcore mode entirely and unlocked everything on Pro so I assumed HC would also unlock lower difficulties.
If that's true then I stand corrected and confused.
