It funny even I didn't shoot both shot each other and I win LOL !
I succeeded in every shoot out on the second attempt (including this Mexican stand-off one). At least on normal both times it was the left guy that draws first. Are you playing with a controller or mouse and keyboard? I think the game is easier overall with m+kb.
2 - Hard - I shot left first then second on right
3 - True West - I shot left first then second on right

Left seem more hate at right but right hate me more if lucky he shot him first I have good chance shot left first then right...
Full time on xbox controller. I think trigger is lag because pull all the way to full...in such short split seconds not easy.
I have done all 3 Normal, Hard and True West :)

I could be wrong, but I'm pretty sure that when your focus isn't high enough for the guy you're facing down, he'll just outdraw and shoot you before you even get your gun out. There seems to be a "threshold"-value for all the enemy characters (at least in the duel challenge). Early ones like Pat Garret seem to need around 80% for you to stand a chance, while later ones (Jesse James, Billy the Kid) seem to require at least 90%.
80% is definetly not good enough on later duels. Try to get both values above 90. Speed 100% and focus 93 - 95% are totally doable with a bit of practice. After the draw, wait a tiny bit for the crosshairs to tighten ("tiny bit" being the keyword here), correct your aim (if neccessary) and don't fire until you're sure you're on target. Sometimes you do get a second shot, but I wouldn't rely on it. If your focus was high enough and your aim was good before the draw, the crosshairs will appear pretty much on target, giving you a chance to almost instantly shoot. You still should wait for the (red) crosshairs to appear though, otherwise your shot will probably miss.
Other than that: Use small corrections on both speed and focus. Overcompensating is your worst enemy in those showdowns.
Oh and BTW: With practice, you can beat the Mexican standoff honorably, killing both guys with a headshot. I just did it.. :)
