Why cant i play insane mode?

I cant' play insane mode even though ive beaten the game on two different characters. Whats up with this?

Does a white skull show up on your character in the select screen?
If not: you missed a level (or more). You need to complete [pretty much all] the levels, and then do the last boss [again].
If so: right when you get into the map press down, it will take you to a new level that says insane mode. It only works if all players are using a white skulled char, I think.

i know i did it before on my ps3 i have a white skull and it doesnt show the availability to go down and i beat the last boss at least 8 times

same happened today on my orange knight, it also doesnt unlock the fire demon as it should.
i cleared it with (white,red,alien,peasent,skeleton) and those are all insane unlocked.

Are you sure you finished every level? If you played online/multiplayer and you died before the level completed and the others finished it without you alive/didn't resuscitate you (example: rapids ride when you die or if you didn't get revived in a boss) then it doesn't count for you, and you have to make sure you do that level over.

Beat the game with the orange knight and go to insane mode also unlocked the fire demon. You may have missed a level. :X Good luck! I am horrible at insane mode hehe

if u skip a lvl and beat the game it wont work u need to beat all the lvls
