By doing this method, there won't be arrows showing the levels you skipped (unless you skip Pirate Ship or Full Moon)
However, it doesn't seem to be your case, as you have already killed him in different sessions if I got it right.
Here goes some of my theories and/or possible solutions (I can't search for more info now, sorry):
It has already happened with a friend who was playing with me. We were playing Industrial Castle on insane mode and he died on boss fight. I couldn't revive him because he was too close of lava and when I tried to revive me, lava would hurt me and knock me back. After beating him, I revived my friend but he didn't get the telescope and didn't unlock Necormancer. It worked after we repeat the level.
When I've first heard about Level-skipping, I did some research to learn more about it. I remember reading about enter-exit Blacksmith, redo Abandoned Mill and dock (walk to the right until you start the Ship level). I've never had to do it before, but who knows?
If you skip levels, the arrow will not show up indicating levels you haven't done, so it will not be easy to spot.
What I recommend at this point is to log in to each character in a local, *single-player* game and confirm that you do not see any arrows on the map anywhere. (Going into a multiplayer game may mean that you won't see arrows for levels you still need to do). Ensure that each character has all of the relics.
Finally, make sure each character has completed all of the mandatory levels {LINK REMOVED}(http://castlecrashers.wikia.com/wiki/Level_Skipping).
If we can't figure out what's up, next plan will be to try to clear those mandatory levels with the "broken" characters and see what happens.

I just wish I knew which of those skippable levels caused the issue in the first place.
You still have one character right? I would try to redo all mandatories with him just in case. The problem may not be in a skippable level, but in a mandatory one.
