Can't get skull on Blue Knight

I beat the entire game with the help of my red knight to rush and without level skipping to make sure I should get the skull but it seems no matter how many times I reset my Blue Knight's data and beat the game, I never get the skull, insane mode, or the Industrialist. I played through most of the game co-op with my orange knight and got the skull no problem. I want to play as my favorite character from my x-box days but this game just dosen't want to give me it. Any and all advice and help would be greatly appreciated.

I'm gonna leave this discussion up for anyone that might need this info but I played through the game as BLue Knight leveling up and playing normally and I got up to Cyclop's fortress and decided to fight the final boss with my Red Knight again and did get it. My only guess as to why this worked this time is that BLue Knight actually lived to the end of the Catfish boss fight and Parade as both of those are checkpoint levels but also just remove you and character icon at the top if you die during both of those areas. All speculation but that is my guess. Hopefully this helps someone a somepoint.

If you skip wizard castle interior / wizard castle take off, you have to do the final battle twice.
And no, you cant just only do final battle and get a skull without doing the rest of the game, I could help you beat the game if you want

I already got it and I played the final battle many a times to see what would give me the skull and not. I'm pretty sure my BK dying on those two levels is what screwed it up for me.
