Any reason to use painkillers manually?

Since "last man standing" exists theres no reason to use them right?

Every time you die you get full health back so its best to die with as many pills on you as possible and whenever you die and you DO have them you get a free kill and full hp

Correct, except you don't get last stand from headshots or from the boss in the cop station and if you are attempting NYM Hardcore you wanna keep your health as high as possible.

We'll I've not been headshotted and the hardest part of the cop station boss is knowing what pannels I'm expected to shoot.

It's also beneficial to go into last stand in Score Attack mode, in fact the game encourages you to do so. You should give NYM Hardcore a go and see if your painkiller use changes.
One other reason to use them manually might be if you have a weapon like the grenade launcher equipped, that will not fire fast enough in last stand.

Last stand is also pretty buggy some times. especially in cover. Max tends to contort in weird ways and fail to bring his weapon around or the game just places the camera in a bad spot that prevents you from hitting the enemy

Sometimes that hapens so you can use painkiller to pervent that but its ok the both way
