What happens if you fail a story quest?
As the title says. Since I am playing on ironman mode, I am being extra cautios. I go 2-3 levels beyond the recommended. Also, I almost lost in the duel against Bai and the thought of not having a monk instructor for the rest of the playthrough haunts me.

Can we redo story/unique quests if we botch it?

There are only two optional Story Quests that are not repeatable upon failure:

The first story quest where you need to find the missing son and Misasa's personal Quest

Every other Story Quest is repeatable if you fail it.

Wait, then I can just enter the quest to find the purple lady, get the chest at the entrance and quit out/die?
Or failing quests also forfeit all items? (I never failed a quest since I reload before it happens)

Well if you don't complete a quest you don't get quest rewards. Which is obvious.
However I have purposely left a dungeon without completing a mission and kept everything I looted.

If you fail on Bai's duel, you can just try again when you get stronger. She's always up for a fight.

Not sure which purple lady quest you're referring to, so can't specifically comment on that.

It's the one for the arcane knight where there is a chest and a house, and when you check the house you get surrounded by sneaky dudes
