There would be a message in your notification area "Some adventurers are very unhappy. Deal with it!" And if you don't deal with it... well, yeah. There's a chance one might leave in the night.
The solution is to not let adventurers be or stay that unhappy. Give them bonuses, invest in better room and board, buy them a drink, give them a holiday sometime. And don't make them do too many missions and assignments in a row. And don't heal them in the temple too often, let them heal naturally if it only takes a day or two.
Managing adventurer mood is a pretty big part of the job of the guildmaster.
Indeed, it's not random in that it ONLY happens if they reach the lowest level of unhappiness.
But once you hit that level, it is random in that there's a... 30% chance per day or something? Max 1 adventurer per day so an unlucky roll can't cost you the game, but still a bit random.
