you only get 1 life....if you die you leave the "nightmare" and go back to the game.

Restart from latest checkpoint puts you back like you just entered the safe house, so you can run upstairs and do it again.

Just the first, unfortunately.
The secret elevator that in Wolf3D takes you to the secret level does exist but also ends the nightmare, like the main elevator.

For the record, the original Wolf 3D was slightly easier even without the health regen. Something about playing it in the new engine doesn't work quite right. Still a cool easter egg though.
