What causes stat bars to permanently shrink?

All bars stat bars start with 6, but some are permanently reduced throughout the game. I'm trying to figure out what triggers a permanent reduction for each stat. I'm halfway through and some characters only have 3 possible stamina or health.

Stam loss - Too long being in Starvation
Health - Major illness or injury but can be restored to 6 by a Physician in a major town or fort
Morale - Too long being in Forlorn
