Kill everything you see, no hostages! Explore the whole map, get all side quests.

If you explore the whole map - that is not a fast level up but the same tedious gameplay.
If you are not playing on Hardcore, skip through regions and jump into the hellish fire of Dungeons with higher Level Requirements than the current Level of your Hero: you might die a lot, but when you kill a higher-level monster, you'll get more XP.
Also, there are items in the game, which increase the rate of your XP gain.
Also, completing Quests bestows Bonus XP.

I usually just do maps from the mapworks that have a higher percentage of exp gained

honestly, just kill everything. the game's exp rate seems fair. I never felt like I was grinding.

on other hand, if you really want to level that fast, then go get a friend higher level that clears everything while you just follow him/her.

Also, you can refight some boss fights.

play with a full group and do the main quests and any side quests if you run into them, don't go looking for them and don't fully explore the maps. This is the fastest way to level up if we're not including cheating or someone rushing you through the acts.

Also if you run out of quests and enimies to kill. Make a server and choose LAN I think and then tick a box to the right that sais reroll world. It will respawn all enimies and will possibly renew side quests. :)

If you've cleared an area map and want to go through it again, start a LAN game and enter the area. It will reroll (re create) the map, randomizing locations and spawning enemies and chests and stuf again. It won't raise the levels of enemies, but it will definitely let you jump a few levels.

Another suggestion is the Endless Dungeon mod in the workshop. Each level of the dungeon has a one-higher level monster set. If you enter at level 78, the monsters on the first floor will be 78. After you find the stairs downward, the enemies will all be 79, 80, etc
