From the above thread:
Duplicate login!
Suddenly, i forced to be logged out cause of server problem. After that, i tried to login, but it said "duplicate login" and i cant login. That problem happened 15min ago, and still i can't login.
--------------
It seems that there are a major problem with any forced disconnection from the server.
You'll get this problem if :
- You join a hosted game and the host quit while you're still in the game.
- If you loose connection to an online game and forcely gets "offline"
- If you alt+F4 or crash while playing online
From what I have read from users who got the same problem as us, it seems that Runic have forgot to put a safe-loggout measure to the server where whenever a player lost connection to the server, he gets logged out. (Or they put one that isn't working at all.)
This might be due to the fact that they made it so that players get "offline" locally whenever they loose connection to the host/server, but didn't put any kind of (working) condition script that gets the player "offline" from the server view.
For players who gets this issue, it seems like the only way to gets online again is :
1 - If the server reset and all player are forced offline. (Note that this doesn't fix the problem if it happens again)
2 - Runic Games made a critical change to fix the issu on BOTH their server and the game and reset the servers.
3 - Peoples start playing only in LAN.
Also, a quick but working way of doing a fix for Runic Games is simply by adding a "force logout" option whenever the server gets a "duplicate" error. It should ask if you wish to force the "other" to disconnect and you'll just have to log-in again once "everyone" from the account is forced to logout.
------------------------
My understanding is that the server keeps a "ghost" of your character logged in. Eventually there is some sort of inactivity timeout that "boots" your character out. I tried unsucessfully for a good 30 min. During this time I exited Steam/Torchlight 2 and everything in between. I added the game to the Firewall and did DNS flush, all of these were considered "fixes" by different people. Honestly I think all they did was buy enough time for the server to boot their characters. I have logged an issue with Torchlight 2 support (still waiting to hear back) I suggest everyone here does the same to make sure it gets looked at. At any rate the day after the issue I could sign in and play online without issue. I can post back when Runic weighs in on the topic.
---------------------
The only current solution is to wait it out till the servers times out your login. Till then you just have to play offline I guess. There is a strong push in the runicgames support forums to fix this issue.

Well, I just changed password on their website. When you're about to log in, click the "manage account" thing, it will re-direct you to their website, there, just manage your account and change your password! Now you can login and wreck some??????
Hope it works
-------------------------
try signing out, then back in, in 2 min. also makesure, that you have only one, steam account, open, per pc that you are playing on, and check and see if you are signed from anther pc.if none of the above is the case, try siging out then rebooting the pc, all the while yelling why god have you forsaken me! shake a fist at some toddlers, then log back in. problem solved. now im off to go flog the??????out of some garden gnomes. good luck to you

In case anyone wants to know Runic did get back to me on July 23rd. Here's the gist:
"I am very sorry for the delay and that you had this issue. It appears that we experienced a problem the other day that caused a small group of players to get this error. We are currently investigating what the cause of this was. I just wanted to check in with you and find out if you are still getting this error at this time? If so please let me know."
Needless to say at that point I was no longer having the issue. As everyone has said here the problem goes away on it's own eventually. They seem to be believe the servers had issues at that time that screwed up a small number of people from logging in. Guess I was a lucky one ;-)
--------------------------------
Issue still persists in 2014 =/ but yeah, changing password forces the server to boot your existing connection so you can reconnect.

TL2 seems to bind to the first network device. You can try reordering your network devices. Control Panel > Network and Sharing Center > Change Adapter Settings > Press ALT Key > Advanced > Advanced Settings - Move the currently connected device to the top of the list. This is a common problem with LAN Games. (If this solution was mentioned previously I am sorry for repeating, I didn't read all the answers. TL2 reminds me of D2 and we used to have a similar issue there before it was patched)\
