How to grind items and level?

I got a main story quest, but my level is too low for it, and my equipment sucks. Where can I grind level and items? Because monsters doesn't seem to respawn.

They respawn after 3 + more zones visited.

You can create a Lan Game and re-roll the world. Are you playing with Mods?

Only by walking will it instantly reset after 3 different zones.

Using Portals and entering dungeons won't reset other areas or vendors unless enough time passes (15 minutes?).

This is the fastest option to reset areas and vendors.

Unless you play with mods that alter the progression, normally if you visit all the zones (you explore the entire area and explore the side quests) you should never be under leveled
