I used to just play with Unearthed Arcana. However, today I changed it up and went with "Torchlight II Essentials" (Vanilla UI) and "Equipment Slot Overhaul EC". Between those two there are tons of quality of life improvements without radically changing the base game.
Why? Because there are so many mods out there. And you can only play with the people who have installed the same exact mods as you, meaning your mod list should be 100% identical.
Take the tiny portion of players that play modded TL2 in multi. Now, divide it by the amount of possible mod combinations.. And you get this result.
The fact that most people play in private games instead of public doesn't help. But heh, i prefer these, rather than those who will insta kick you when you join their game.
If you want to play multiplayer ,either do it in vanilla (where it's not that active, but you can still find some games in Veteran/Normal), or do it with people on your friend list in modded
Synergies changes almost everything about the game and includes many small mods swallowed inside one big overhaul.

However it is aged since it does not include some of the latest game improvement mods made by "doudley" which means it does not contain all the potential from the Workshop.
Nevertheless it is the oldest largest most respected mod so many people tend to choose it.
It's best to make friends on the Discussions and just start agreeing on which mods you wish to use and start playing together.
