it's easier if playing on Veteran or a lower difficulty.
Elite can only be beaten by very specific skill sets without dying too much.
Hardcore Elite is extremely tough without maxed defenses (75% Dodge, Block, All Damage Reduction) or alot of running from enemy attacks while investing into high powered Strength or Focus and some Dexterity.

Boris the Enchanter boosting high leveled gear is very important for survival with high stats.
Yes, especially when trying to level a Class to 100 (may replay the story multiple times) and then there's the Mapworks which is the place to grind endlessly for useful stuff... it may even be of interest to grind there before moving on to higher New Game Pluses just to be prepared with the correct equipment.

These are probably the 3 best guides for anyone who is serious about grinding and beating the hardest secret challenges the game has to offer on Elite or Hardcore Elite.
Skulls of Limony (or Eyes of Grell) are crucially important.
I have yet to defeat NG+ Tarroch's Tomb on Elite difficulty after 500 hours.

Also consider "Grim Dawn" as it's a good second game to have with "Torchlight 2" since it's a very different style with lots of depth and it actively competes to be the best in the genre.
Yes, however the ending does not stop the player from continuing.
Once the "Netherlord" is defeated the game activates the "End" scene and then the player can return to the Minehead Town.
At that point the player can either grind in the Mapworks (which unlocks it for future New Game Pluses permanently) or go into New Game Plus (there's 5 tiers of NG+ all based on what level the player is at).
New Game Plus and the Mapworks can be played endlessly.
The game does cap the player at level 100 and enemies cap at level 120.
The only exception is the New Game Plus dungeon called "Tarroch's Tomb" where foes can go as high as level 200 as a secret challenge.
