What would be the most lore accurate difficult?

Grimdark demands that you max the difficulty and Git Gud (TM). Unless you're Matt Ward, then set it to easy, to properly experience how Ultramarines are clearly superior to everything.

Whichever setting allows you to scythe through enemies while still having to consider them an actual threat. If you're standing on a ledge overlooking an arena of alerted enemies and surviving, you're on the wrong setting.

(3rd or 4th difficulty imo)

In the lore, a couple Space Marines can take out an entire planet of heretics, so probably easy.

lore wise? lore wise you'd have to have Ragnar or Calligar level plot armor to take on hundreds of chaos space marines and several greater demons of chaos all by your lonesome.

Please dont do this, i know how important the lore is for you lore nerds and i respect it, it really do. But this is a classic boomer shooter and not the next turn based tactics game ;-)

If Matt Ward had any saying, everything you choose would be easy mode, because after all, in this game you play an Ultramarine, even a best of the best of the best.

I played for around one hour and decided to change for the exterminatus difficult, normal was too easy and on exterminatus fighting against chaos space marines feels like an actually threat
