As far as campaigns go, you only need to complete them with Batman, and only the original ones. It doesn't matter at what point you complete them.
Yep, you have to do all side missions so AR training too. Its same difficulty as normal game, no change.
Both the Riddler Challenges and Campaign Challenges are completely independent of NG+ progress, they are always the same difficulty no matter what the difficulty of your campaign is or whether or not you have unlocked NG+, and you won't have to repeat any of them. Also, even if you start NG+ you can still also continue your previous game from where you left off.
