Press Windows Key + R to bring up the run dialogue box.
Type "regedit" and hit return.
It is recommended that you export a copy of you registry before you begin, in case of any mistakes. Select "File" then "Export" and choose where you want to save the backup.
One done you need to open "Computer\HKEY_CURRENT_USER\SOFTWARE\ubisoft\prince of persia\1.0\engine"
Locate "ScreenResolutionHeight" in the right hand window. double click and select the Decimal button under base.
Change the Value data to "2160" and select OK
Locate "ScreenResolutionWidth" below and then change the Decimal Value Data here to "3840" again selecting OK.
Close regedit and when the game launches in future you will get 4K resolutions.
Warning - Be very careful when using regedit. Editing or deleting the wrong keys can mess up Windows. Don't experiment, and above all remeber to backup the registry before making any changes.
I followed these exact instructions, and this did not give me a 4K resolution I could apply. Note I am using DSR to achieve 4K currently on a 1080ti.
However the above will allow you to run the game at 4K resolution without having to go back into the options to apply any settings, as it forces the game to run at that setting. If you apply new options within the game menu after setting the registry options the game will undo the registry changes.

Hello WitWolfy,
Thank you for reaching out.
I'm afraid the original Prince of Persia does not natively support 4K resolution, while workarounds such as above may work in some cases, there's no easy official answer.
May we please know which of the two suggested you tried and what brand GPU you use (AMD / NVIDIA)?
Thank you!
