Mouse Lag

Is it just me, or even despite mouse smoothing being off, it still suffers from heavy lag?
Its a bit like playing the game while drunk, it feels like my aim isn't going where i want at the right time.
EDIT: seems to be releated to smoothing options in the Controller menu aswell, disable those and it should be reduced. It still feels a bit off, but not as nearly as it was.
