Plagues will always be around the corner, you need to avoid being full of them, but you will seldom have no epidemics in a large nation.
2. Build any and all building that have a positive health modifier (these are not always in the "blue" category such as cemetery is "orange")
3. If you are fortunate enough to have a character in your court that has a bonus to health or fighting the plague you should assign them as a Peer of the Realm to that province
4. So far in my play thru as England, I'm finding that the reconstructed aqueducts are bringing the most bang for my slot in the health regard, however I want to point out the Wise Woman (I think that is what she is called) give the Natureopathy bonus (+15% health per health building) or something like that which makes a HUGE difference
5. While it may be tempting, try to limit your growth and how fast you grow your population to some degree. I've been toying with what works in Kingdoms vs what mix I used in Empire (3/4/0/max pop spreads). For Kingdoms, I'm basically putting all peasants into Agri, then a mix of peasants & Freemen in Ind/health until the two are about even, then I put my priests in piety and nobles in stewardship unless I'm raising army and then put one or more into equipment. This has, so far, kept me at a decent rate of population growth to where I can account for the health needs via just building another health building. You can tell if you are in danger in a province if the Population/Religion/Culture banners at the top of the screen when you go into each region are black or orange. If they are black you are ok for the moment. If they are orange you are in danger of a plague/heathen spawning and you need to take proper corrective action. If you hover over the banner it will give you a break down of numbers and advise to some degree on how to correct
