There is a secret quest that can be started by interacting with an old lady and some thugs outside Kilos. But when I go there, no-one is to be seen. One post on Reddit said I had to beat the story. Is that true?

Don't remember the quest name but I do remember starting a quest like that after beating the story so it's likely the case.

Can't really say much either. I did it after the main story as well. When you leave Kilos, keep an eye on your right when travelling on the road for a group of 4 people tucked between natural formations. It might pop before the very end of the main story, or it might be immediately after.
