Heathcliff distorted because he succumbed to them. After beating him up, he just kind of just went, "well, it is what it is". instead of overcoming it. This was why he came back to normal after distorting, he just stopped letting the thoughts consuming him.
Later, when he found out about cathy's true feelings, it was a load of his back. a revelation sort of speaking. This would be equal to having Lowell come out along side the rest of the lui association saying stuff like "hey we are okay actually, let's stop and go home" for xiao in midst of her transformation.
It's pretty straight forward, his burden that was the cause simply disappeared. He didn't succumb or take control over it.

No one expected the "Vergilius" intervention for example. At least not this early in the Story.
Though I do agree with some others here. Manifesting ego means getting over yourself despite the fact you're about to distort. If you've read Vergilious's story, you'd know we get a first hand account of what this moment looks like from the person's perspective. Heathcliff definitely wasn't that, they gave up and returned back to human after. The closest they came to manifesting an ego was when the entire world changed to that graveyard during their prolonged attack against their other self. A part of me WISHES he'd manifest, but one new mechanic/toy at a time I guess. We got new powers to help us in future boss fights assuming they start increasing the number of activations some time soon. Right now I never want to use them because no only do I not get to use another for the rest of battle, I don't get to use another for the rest of the day.

It's what Vergilius used when he transformed mid-fight.
An Effloresced EGO is massively powerful, but it can only be achieved when the user themselves, without any external influence, overcomes their trauma. Which Heathcliff didn't.
It's also permanent. It can be used nearly without limit. It's not like our current EGOs which can only be used for 1-2 attacks.
