hopefully you brought along a unit that can resist pierce or at least not get deleted from her one-sided attacks (i used K hong lu).
if you're talking about the last boss,
have ishmael use snagharpoon (whatever her base ego is called) against the high-rolling mass attacks, with the +3 power she gets from being in negative sanity her floor should be high enough to always win clashes, and she should get just enough sanity from winning clashes to be able to spam it for the first two phases while still being under negative sanity
let your other sinners slowly build up sanity from the other attacks.

3.(IMPORTEN) try farm ego resources that the big clash ego need it posible if not you can try reset bc phase 2 attack is the real problem and try hold the sinner alive it long it posible
Much like the fight in 5-30 against the B.B. Ishmael cannot die during the final fight as long as there is at least one other sinner remaining
With that said it's anything but an easy fight (Though they seem to be nerfing it, which while I personally enjoyed the difficulty I can understand the decision) and you may be retrying a decent amount
Maybe take 1 less sinner into the fight so that another unit can have an extra skill slot, it's risky depending on how many you decide to bring instead of the max amount, but it can help make certain fights a bit easier! Especially with a certain currently-immortal-during-the-fight sinner
Having 0 sanity while the other side ramps up every turn is discouraging.
Nerfs have already being announced so I guess I'll wait.
Try to kill the other side's units first before they kill one of yours and snowball down from there, I had more issues with this one instead of the "Final" final fight

also refill your sanity as much as you can since she loves to drain it. on one of the phases, i think the final one, she tanks your sanity every single coin that hits her. and assume that every coin will roll max value since she is legitamately just always at 45SP
