I think it Ruina starts about a year after the White Nights and Dark Days at the end of Lob Corp, and it doesn't sound like the Cleanup crew had to deal with any distortions in that time. Perhaps they just mistook them for the usual "zombies" they had to fight and were just lucky they didn't encounter anything too dangerous?
I don't think the Cleanup crew that went to the library was all of them, at most it would have been only one station's worth. Also the half a year between the ending of Ruina and Limbus is more than enough time to locate their missing staff.
The horrors of a WARP train are likely too long and drawn out to properly trigger a Distortion; people get acclimatized to the fast and furious swan boat before they can experience the proper emotional shock to trigger one.
That's not to say W Corp isn't in trouble, though. Lobotomy Corporation provided power to the entire city, and there's definitely a (literal) power struggle brewing without them..
It needs to be something that shakes your core value or crushes your ideal heavily, to the point that makes you dive deep into your very being and then either lose yourself to become distortion or overcome the hurdle and manifest EGO.
Like how in LoR, Yan distorted because his ideal and world understanding are completely destroyed. He thought he had free will and that he was choosing to do things on his own mind, only to be realized he and the entirety of his surroundings were just puppets all along.
Or Xiao manifested EGO by realizing she has been too obsessed with revenge to the point of losing all those she held dear. Decided to overcome the grief and gain power to crush us with serene mind.
All those floor realizations were the same, forcing librarians to overcome stuff related to their past and ideals in order to gain more power.
