You didn't lose it, It's just in your notes somewhere

It's a quest item, so it doesn't go to Inventory.
It can be found in Journal > Unique items.

You get it from Santa Maria, yes. If you haven't been at that location, then you'll get it there. =)

As this Discussion was pretty clear, I have still not been able to locate my sextant. I did pick it up, but have searched through my unique items (at least 3-4 times. My inventory (3-4 times) read every page and looked at everything individual item in journal at least 2 times. But alas i have still not found it

Are you sure you have picked it on Santa Maria? What story quest are you currently on?

Open your Journal.... click on the Diamond Icon on the left... at the top you have left and right arrows ... click on right arrow to give you a list of Artifacts.... if you got it, it will show up in there.
