There are 5 difficulty levels: Very Easy, Easy, Normal, Hard, and Very Hard. They define your vitals drain, damage from animals, chances of curing diseases, negative effects, etc.
Apart from those, there are Survival modes that define the number of fainting available till permadeath:
Tourist: unlimited number of faintings
True Survivor: 12/9/6/5/4 number of faintings (depends on the difficulty you choose)
Iron Man: one life option
If you enjoy challenging gameplay, I suggest you check Thakard's playing Survival: Fountain of Youth on Very Hard: https://www.youtube.com/live/jilRh2dQsdY?feature=share
Hope that helps!
Awesome to hear that, hope you'll enjoy the game!
