Report a translation problem
1. You must throw the two fights which trigger the introduction of Succubus Eve BEFORE triggering the second event (the one on the "far right"). If you don't, she will never appear in the second event and you will be locked out of the good ending. I also suspect that the second throw may not actually be necessary, but that is not a priority for me right now, because...
2. Even after doing all of the steps detailed in the OP and item #1 of this post, this will still not guarantee the good ending. Regardless of whether I chose "Return" or "Stay," Eve will be unable to harm Succubus Eve in the post-boss cutscene and the dream will collapse before she is defeated, leading again to the standard ending. This is also true whether the final dungeon is 100% completed or not. I have a sneaking suspicion that the true ending will also require 100% completion of all other dungeons in order to trigger it, since I had to go back to a previous save file to remake my "Return" or "Stay" choice. I will edit this post once I verify this hypothesis or otherwise come across any other hidden conditions which must be met to unlock the true ending. Wish me luck! :)
Update 21-Apr-2025:
So here are the missing bits of information: The correct option is "Stay with Eve;" I didn't try the other option since I saw little reason to do so (if anyone wants to verify that, please share). I was also right in that you need 100% in the previous dungeons in order to unlock the true ending. You also don't need to die the second time. Finally, the last dungeon itself does not need to be 100% complete to trigger the ending. So I think that's it. Hopefully this will make the achievement less of a hassle to get. Have fun! :D
