I ran through the game again, following all the same steps, but I think I may have simply forgotten to throw away the second bloody doll you get when you open the tatami mat in Maina's room 203. So, maybe, make sure you throw away the dolls as soon as you get them and you'll get the good ending. That's the only thing different I can think of doing this time around. So, anyway, if you're having any trouble, make sure you do what's in the spoiler text.
You have to cut the chain every retry
After discovering the chain, I did cut it each time. It still took 3 total tries to get the good ending. It must be something to do with the dolls, because just cutting the chain didn't do it. For me, anyway. Could be a bug.
Now, the game's save feature is kinda finicky, so saving and quitting at the wrong time can make getting the good ending anywhere from annoying to impossible.
For example, even if I technically met the requirements for the good ending the first time I beat the game, I couldn't get it because the key to Maina's room disappeared from my inventory after quitting and reloading on day 5, so I could not get the newspaper clipping in her room.
I've also noticed some miscellaneous weird stuff as I load my saved game, like the monk sometimes "forgetting" he blessed you.
Thankfully, the game can be beat in about 20-30 minutes once you know exactly what to do, so it's not much of a hassle to speedrun the good ending and circumvent the save system's wacky shenanigans.
Well, that's weird. I haven't played it again since I finally managed the good ending, so I haven't gone through with any variations or anything. I just know that when I threw the first doll away only I didn't manage the good ending, even though I went through every other step and cut the chain, etc. Like I said in an earlier post, afaik the only difference between the two playthroughs was the doll thing.
And yeah, I always did a full playthrough each time. I never even tried saving and quitting, so I know that wasn't my issue :-P
