I have 100% had whisk(e)y detract from my relationship score. I distinctly remember this because it was the first and only time I used it.
Yeah, there are so many ways this system could be designed to be less of a straight-up random middle-finger to the player, and I'm not sure how it got through EA like this (though I default to my position of "the devs designed for players who'd unlocked everything and had to ramp up the difficulty of everything", which is a failing of many EA games). As it stands it's a major balance issue getting in the way of fun.
You do not seem to realize that there are multiple types of whiskey items, the default one which is unlocked from the start does not reduce relationship score. Neither does pipeweed and it's also unlocked from the start.
That sounds like you aren't focusing on relationships then. Affinity is just as important as stress in DD2. Or maybe you aren't managing your stress well enough, and constant meltdowns is tanking your relationships.
DD1 had a hint of this with e.g. religious charcters having a bonus from correponding skills or trinkets.
I'm not sure it would be enough of a punishment for losing the relationship game. Plus I feel like it fits thematically. That things you do will piss off your "adversary" even more.
Unlock more items using candles until you unlock the whisky barrel, they'll appear in the store from then on + have a chance to be donated/found through caches, etc - Spam these + whisky and you'll be walking into runs with a full screen of positives.

I can't hear you people whining about it over the sounds of my quadruple positive relationships rekking the boss with my gigachad MaA and waifu GR, among others.
Getting cursed skills isn't the end of the world either. The debuffs the curses give can be resisted or countered with skill effects. The stress can be healed. You people act like once you got some cursed skills the game alt+f4s and slaps you across the face.
Learn the system and you'll have more fun with it.
