It is possible to succeed at every every expedition, once you've unlocked more skills for heroes, paths and more heroes in general you'll have alot of options to make a team specifically to slay the end boss of each confession.
Only Confession 1 has 2 regions then Boss. Because each confession has a narrative attached to it, they end after the boss has been defeated. An endless mod in DD2 akin to the endless mode in DD1's Colour of Maddness would be a cool addition to DD2 - but currently doesn't exist.

No, after the mountain boss fight the run is over. As Chester said, act 1 has 2 regions prior to the mountain while all other acts have 3 regions.
