it's best if you stack DoTs on the boss since the two lungs can't get DoTed.
have someone with taunt tokens on your tank or dodgers.
Just do the mechanic of dealing 12 to the front lung and 9 to the back when inflated and you're good. Bring 2 people who can project dmg to rank 4 and 2-3 for the front lung. One dotter on the main boss helps a lot too.
It's only worth (in my opinion) when you deny the lings as it is. Denying each inhale, I had times where by the time the boss is around 20% hp left, he had one or both lungs killed.
And if you find that milk soaked linen item during the run, the one that removes blindness tokens, hold onto that because that boss applies those to your team. So it's invaluable to have a method of removing those. But this is true for most bosses in the game, they just love blindness tokens.
IIRC the boss is succeptable to blind, so if you can lock down his lungs and have a 4th hero put blind on him that can help neutralise him.
As others have stated too DoTs are your friend, stack DoTs on the middle and focus down his lungs and run damage mitigation and let the DoTs do the heavy lifiting.

Without lungs, the only way it can threaten you is by landing a combo token into a "Dying Light" crit - and even if that happens, it will struggle to capitalize. Otherwise, its only hope is to land blinds, and for you to miss when it matters most, which can be resolved in other ways.
Just like every other boss in the game, DOTs are strong against the Sigh. If you can pop lungs while stacking damage over time (eg, using Plague Grenade to tap the back-lung for a few points of damage while applying blight to the main body), life will be much easier. If you go for pure raw damage, you must contend with the frequent blinds + Block tokens. There are methods to help with that, too, but it's easier to bypass its defenses and let it melt to death.
On that note, consider items and trinkets that cure or resolve debuffs. For example, GROG on a fast hero can convert blinds or vulns across the team into something useful instead, but if you lack options like that, simply carry throwable combat items on heroes who will be attacking most frequently. That way, the blind tokens will be wasted on the combat item, making it easier to land your skills. Failing that, milk-soaked rags, debuff resistance, holy water (etc) are a handy alternative.
Ultimately, it's a projection test. If your team lacks the range and flexibility to adapt for lungs, then you will likely be put on the back foot by burst mechanics, making it harder to recover.
If you prefer a visual example, here's a random run from day 1 of the DLC with a team at baseline damage - back when I had no idea how DUE worked, so I'm also playing poorly - but because I intentionally picked 3 heroes for each lung at a bare minimum, the only issue was blinds and one Dying Light, and as you can see, the boss couldn't even capitalize when things worked out for it.

The comp your running has means to handle the boss, what skills you got on them?
Step 2: Attack only one lung when it inhales
Step 3: Attack middle when no lungs have air or when both have air
Step 4: Win :)
Also as stated above don't try to kill any of the lungs. The whole boss has a lot of health and you'll do better to spend as much dmg as you can on it and not its parts (outside of deflating them of course).
