So, who's the killer ?
I saw 3 endings, and i am not sure to understand all the game tries to say to us

Same. I spent 10 hours getting one ending and just uninstalled the game. By the time it was over, the only plot I felt successfully resolved was a "C"-plot concerning a body. I don't have the time, nor inclination, to play through this multiple times just to understand the primary plot.

Maybe something was lost in translation, but I'm afraid I'm not going to be recommending this game to anyone, ever. It can be polished, pretty, well voice-acted, and high-quality, but if your story is crap, the game is crap.

There is no killer. Greene and Kovalev had hatched up an elaborate murder-suicide plot to kill themselves and take down the society with them. The reason was that the desynchronization syndrome everyone suffers from inevitably gets worse the longer people live and there is nothing anyone can do to stop it. The signs of struggle with the Greene case is his desynchronization syndrome acting up. The voice of the killer you hear during certain parts of the game is only in James' head.

I originally thought that might be the case, but then why does Salma get into an extended shootout and conversation with you?

I think there are some parts of the plot that are lost in translation.

I suppose she is simply defending herself because she was pregnant and in a dangerous situation - all the riots and destruction around her.
While the protagonist is making his way to her hiding behind the blocks, he is talking to himself, to the hallucinated murderer in his head. Since he imagines the murderer, he is probably shooting in Salma's direction, so she shoots back. In the end, in the good ending, we can drop the gun and let Salma go.

At least, this is how I understand it. I only understand it now after I played someone else's save file, tried different sequences from what I experienced, and watched several videos. So I agree that the ending, if you walk out straight from the game, is a bit confusing, to say the least.

What puzzles me is if there was no murderer and Salma controlled herself, why the heck was she at that plaza, pregnant? I don't think it was explained.

Salma was at that plaza for Greene's memorial event that was arranged for the city's elites. The rioters went there to "eat the rich".

i dont want to sound critical, but story tellers need to move away from "it's all in your head" plots.

I think they're just using a broader definition of "it's all in your head" - in this case, their issue is against plots that only exist and are driven by the protagonist's psychosis. I don't agree, and the movie industry's shown many ways that mental illness of a protagonist can lead to compelling stories that leave the viewer guessing (and this isn't the first video game to do this either, Hellblade Senua's Sacrifice had you playing as a schizophrenic Norse woman and everyone loved it - can't remember opinions on the recent sequel but that's neither here nor there), but I can see why someone might feel a plot reliant on the protagonist's hallucinations or dreamstates is frustrating.

That's not at all what this game is though. Throwing everything that deals with ptsd or mental anguish into the same bucket throws out like 90% of media.
