The number 1 issue with software and their implementation is the false belief that they are infallible. That's why when the cops pull up the records and see X they believe X. When they get in front of the judge, judge believes it too. Then dude sits in jail. Then the department realizes a bunch of errors are in the system and dude spent time in prison for nothing.
