Must be a bug because most doors do have holes and windows.
Have you ever experienced something similar when playing the game?
I've encountered a few bugs where the game will glitch on the furniture item you place, so you have to manually save and reload to it. Never missing door holes/windows though.

The game runs very smoothly with the highest graphics settings, but I've noticed an issue where the doors and windows look like they're part of the walls, instead of being hollow.
