It's totally possible to do everything right and still lose.
This is why you save.
I don't think anyone has definitive proof of what exactly changed from Hard mode, but I can tell you Hard mode does not feel anymore difficult than the original PS1 game by any measurable degree.
If you have sufficiently leveled Pahn to at least 32, upgraded his weapon to lvl 12, and given him the most up to date equipment, including two guard rings, it's virtually impossible to lose.

Even though Special is the counter to Attack, you don't want to make the exchange, just eat it with a defend, unless he attacks you 8x in a row, you're going to end up winning if you get all the other calls right.
It's not even worth arguing over. Just save and do it over and over til you win. You can just defend every round and win eventually.
I did use the old trick of fighting the war battle with just magic attacks. It was an old rumor that doing so increased Teo's chances of going for Desperate Attacks. I don't think there was ever any data-mining to prove that affected the duel at all. I always thought it was just a bit of a troll by someone, since losing mages isn't hard in war battles when they're countered. Might be the placebo effect, but ever since I started doing that, I've never had to re-try a battle against Teo with Pahn. Then again, I've always leveled him to at least 30, maxed his weapon level, and gave him the best armor possible for that fight.
I've looked up like 8 guides for this duel and the previous one and NONE of them had all the proper phrases, the one posted earlier in this thread was the closest yet, but each one had at least 5 that I saw that were not on ANY list, some not even close. Whoever making these guides on other sites need to step up their game lol.

Go to the blacksmith.
Equip him with the best possible equipment.
at least 30+ LEVEL.
And READ what Master Teo is trying to "say" what hes going to do next :D
