The vision becoming blurry and a sound playing is such a Personal Scare that me and my buddies refer to as "being possessed". We call it that, but it literally has no meaning or bearing other than a "boo" effect; it does not indicate Ghost activity or vicinity. It can happen no matter how far away from the Ghost you are. As far as I know, it can happen at any time and any place so long as you are inside the building. It can even happen after you already exorcised the Ghost. Other examples for Personal Scares are Grandma in the Nightgown, the Ring-Ghost crawling towards you (although that exists as a Map Scare in some places as well), or flying Heads that explode when you approach them.
Ghosts talking directly is a type of universal Ghost Scare, but they too come in a form of individuality. Sometimes (though rarely), only one person will be able to hear the Ghost talk, and even if everyone hears the Ghost talk (which is most often the case if they talk), everyone will most likely hear different voice-lines. This is normal.
