Where are the refined Ingots and source stones and how does the hammer of golem work

I'm in chapter 2 i believe and only have two refined ingots and i swear I've picked up so many source stones yet I have legit zero, I've only used two in my time playing so where the hell are the rest???

Also how the hell does the hammer of golem work as I'm hitting it on the ground yet nothing happens, I've killed knight dude out back and tried all the things the weapon says to activate the skill but nothing.
