I managed to get all the journals, but missed the survivor which needed the key from the beginning of the game, but managed to get that one in ng+ but didnt get the achievement for it, do i need to get all the rest all over again in ng+ for the achievement to unlock or is it currently bugged?

It's all one run. If you have an old pre-NG+ file there is a mod that will pop the achievement for you without the locked gate journal. And there is a mod that ditches the key being needed.

Collection items like records or journals do not carry over to NG+ and you need to find them all again.

How far are you? Do any of your autosaves go back to the pre-NG+ file?

yeah I got an autosave that goes back to Abyss

Okay. Load your pre-NG+ file. Make a separate Save [if you can it has been a hot second since I've played the game.]

Pick either of those mods and download and install them. I'd pick reduce for fastest result but first one if you really want to actually pick it up for any personal anxieties.
