Achievements not popping?
I just got the last journal entry, I have them all (double and triple checked) and no achievement popped (and no I am not using mods).

I've actually had to manually force a few of them to take, because they wouldn't pop up, even after reload. For example the level 99 achievement didn't pop.

Same Problem. All journal entries found, and no trophy.

Are you sure you don't have the trophy? I turned off the Steam overlay so I don't get notified of them, but I still get them. Check the list.

No, I have steam overlay on. I reloaded several times. I got 'The End' one right afterward though, so idk what the issue is. Some are just bugged. I had to force the journals one too.

Duh, I may have one hidden journal entry missing, the one where you have to restore the painting. As this may take longer I will report back when I did this one.

Strange though that the gestral in the village already told me wow you found all of them... oO

there are 4 hidden journal notes, 3 unknown + 1 Survivor

in total you should have 49

There are secret entries that are not shown in journal, i recommend using some guide from YT or some other source, i had the same problem, and turned out there's renoir entry too which i didn't find :)

Well you can use steam achievement manager, if you are sure you got them all.
I used this guide, i'm linking it because i found some "guides" online that were plain wrong or had typos etc:
