How do you play it? How should I play it? Is there even a difference?

I play exclusively keyboard and mouse - I've finished the game and am working on NG+, killed all of the optional and super bosses. The only thing that's an issue is occasionally the keyboard just brain farts and doesn't respond. When you're parrying/dodging, this can be a fatal issue. So when I am fighting, if the monster is about to attack I will tap parry just to 'wake up' the keyboard so when I parry it actually parries ...

Try rebinding parry to your mouse. I've been playing on my controller, but there is one attack where I rebound parry to RMB because I can click faster than I can press the right bumper on my controller.

I play on M + K except for the volleyball minigame on the hardest difficulty.

M + KB for me, better free look with camera and I feel much more comfortable.
