Honestly never failed it so I have no definitive answer for you but I do remember the skill description specifies a “risky” attack and another later skill does damage her on fail. So I imagine it’s likely intentional.

I could have swore I remembered risky in the flavor text. Ah well, no idea then.

It'll tell you on the skill description itself, otherwise you just lose out on damage and/or special effects for failing the QTE.

Of the many times I used it, only once she self-inflicted horrible damage. I suspect I had got one or two of the QTEs wrong.
