I have also run across this. I'm confused by your response: why are you asking US if we think it's a bug? We are trying to ask you/the community if this is a bug or what the functionality actually is.
+1, she probably misunderstood the OP, though I hope it's not rude of me to guess that. She is quite busy checking all the posts on forum and replying to them. There are many Wolf den like structures in seeds which are empty for everyone.

I had actually assumed that they were inactive for optimization purposes.
The wulfar dens that are inacive when you first spawn in, does the inactive ones ever become active or are they permanently inactive?
Well, with the recent addition of map generation, I think it would depend now on the number of mobs you select. Though I started a new game after this update. I haven't tested anything on an existing save.

I do think there may be some inconsistent behavior with wolf den display. I've killed Fenrir multiple times. One one occasion, I got a message that the den has been cleared (similar to clearing an enemy site) but on another occasion I did not receive the message and the den stayed red on my map.
Seems a good rule of thumb is if the site is grey or inactive when you first stumble across it, it may very well reactivate later so respect the distance when making new settlements.
If you are just watching the map to see if it changes back to active - it will not.
You have to visit the location for it to update the status.
