best ratio is 5 iron 15 coal and keep the heat at 1600. Not above 1600 or below 1200

Because it's a cube you have to hit each side. The turning buttons let you get to each side of the cube. There is a visual indicator of slag, the black material on the cube, but because the sides are close it's difficult to tell. I rely on the sound.
The whole cube has to be cleaned of slag before the orange bar depletes, which is indicating that it's still hot enough to work. If you don't get it done in time, you'll get less useful metal and some slag.
Be careful once you take bloom to the anvil to be worked, though, because you only have one rotation button to turn a piece you're working on, which is 'R.' If you hit 'T' before you're done it'll quench, that is, throw it in the water, and it'll ruin some metal.
As indicated above this is part of the in-game tutorials.
There were no prompts. It's early access people don't get your pantsu in a twist. It's a good game but clearly I've played it enough and should wait for a bit more polish.
My other question - when an iron tool breaks, is there a way to repair it, or do you just have to build a new one?
The prompts are in the lower corner. They show up there every time you do it. I'm afraid that it's not a bug and won't be patched. The game has a lot of bugs, but that isn't one of them.
You'll be waiting a long time if you're expecting that to change, most likely.
I think slag is just waste, at least I haven't found a use for it. But when an iron tool breaks, you get the metal head of the tool back and can make a new tool, so the bulk of the iron never gets wasted, just the wood and (sometimes) a bit of iron via nails.
You've probably figured that out by now but this post still shows up in google so I figured I'd let any future googlers know.
I really enjoy smithing here although I hope it will be altered a little. Cube things is quite ok, but making some tools should have more variety, also I don't think nails should be hit from all sides maybe just 1 or 2. Bigger things could be even like cube 6 sides and small/medium 4 sides.
Would also love if you could "sharpen" tools and weapon to give them some bonus depending on your smithing (imagine kingdom come deliverence minigame).
Thing in this game to gradualy let your npc take over some tasks from you but at the same time it's not just "press x" 10 times to do everything. You have to plan, and be observant whole smithing is one big series of diffrent challenges that you can do on a side monitor while watching a movie (or not at first at least).
Also those metal heads if they're forever then this really helps out i broke a hammer found a hammer head thought I made it by accident but now it makes more sense.
I'm at the place where I start giving iron tools to my villagers so it will help out a lot.
