Thing is, none of the Fireflies, including Dr. Mengele, ever bothered to let her know that they were going to kill her so she could *really* make the choice.
Later, in Part 2, she DOES say that she would have, but nobody asked her then, which makes them utter monsters, in my opinion. And I completely understand Joel's choice. If somebody were to tell me "hey, we're going to kill your child, but it's for the Greater Good. Maybe. If it works", they'd become really well acquainted with my Texan arsenal in a hurry, and not in a good way.

I've just finished my grounded playthrough and even though I remembered a lot from my PS4-experiences, the enemies felt a lot smarter.
As of getting flanked a whole more often and annoyingly shot from beind. XD
When you save Ellie, you must shoot the doctor.
I am not sure about the nurses, but I let Joel make no compromises and shot them too. :O
