Is this a Film Grain issue? I always turn that off, and Depth of Field (Gameplay) and Motion Blur
Are you using DLSS? It particularly doesn't work well with that, I believe the problem is it's essentially a low-res post process effect that really messes up reconstruction. For this reason DLSS/FSR in this game, especially in dimly lit areas, is very poor.
I am using FSR, maybe I will try it with it off and see if it is better. May have to go down to 1080p or 1440p with lower settings.
That is indeed it. FSR/DLSS in combination with the flashlight produces significant artifacting, especially at resolutions below 4k:
