Report a translation problem
The game requires a minimum of 4GB of vram. Could it just be because of the 12 ram?
16GB of system RAM is the minimum requirement. You have less than that. So that is most likely what could be causing this. Or, since you can play but than it freezes it could be a thermal issue. Have you waited for shader compilation to stop before playing?
Also, 12GB of RAM is odd. Are you using 3x 4GB sticks? You may have bad memory.
When the game freezes is it a complete system lockup? Or can you get back to desktop? Have you tried alt+ctrl+del to sign out and than back in? Or bring up task manager to end task?
The quickest and easiest things to try and troubleshoot this issue would be to do a clean boot and run DDU to clean out display driver. Than install latest.
Running DDU will remove previous driver/s completely and ensure a clean install of newest display driver. Like its being done for the first time on a newly installed OS.

A clean boot will remove any potential conflicts with other software running in background. Disable all non essential services, programs and tasks from starting with PC. So use services (services app or system configuration app), tasks (task scheduler app), and startup items (task manager) to disable them. Check all 3 areas. If anyone is enabled that software will run and could be causing conflicts with games. This will also help free up system resources in general.
Check event viewer? See if a crash report is being logged. So open event viewer app. Go to windows logs>application. Look for The Last of Us at time and date of crash. If a report is even being made. Look for faulting module name and path. Its at bottom of page under the general tab. As well as checking system logs at time and date of crash.
